Digital transformation, interoperable systems, and data sharing initiatives have resulted in state health agencies having more data than ever before. However, challenges remain in organizing data around an individual and family to deliver person-centered coordinated services. Given the disproportionate impact of social determinants on the HHS program population, HHS agencies in particular need a single, unified view of program participations across healthcare, social services, and community-based services.
 
This session will explore how a consortium-based Master Person Index solution serves as the foundation for a “Whole Person Data” view. Learn how to leverage real-world referential data in coordination with your program’s data to create a singular view of an individual across HHS programs. Additionally, this session will share how HHS agencies have used referential data to gain insight into SDOH risk factors and further improve care coordination for vulnerable populations.
